What is an IUI?

In IUI (intrauterine insemination) is a common infertility procedure performed by a RE or OB/GYN and is scheduled for women with PCOS (and women with other fertility issues) after a heavily medicated cycle (Clomid/Femara and injectable medications). Tests are performed prior to the IUI to insure the patient has at least one, if not more, mature follicles (ultrasounds and estradiol levels as mentioned in the previous posts). The IUI is often used when a patient has clearly had mature follicles but still is unable to get pregnant. It is a good way to "bypass" a hostile inner environment and/or hostile cervical mucus.
Prior to the procedure, your husband will be instructed to visit the lab and provide a semen sample. The lab will then take this sample and put it through the "washing" protocol. After the semen is washed, it is brougt to your exam room. The doctor starts with inserting a speculum, just like in your annual exam. Then using water or a sperm friendly lubricant like PreSeed, the doctor inserts a very thin and flexible catheter in the cervix. Washed sperm (Sperm washing allows your husband's sperm a higher chance of survival and fertilization. Washing separates sperm cells from semen, which helps get rid of dead or slow-moving sperm and chemicals that may impair fertilization) is then injected through that catheter and into the uterus. IUI's are usually performed around 24 hours after your HCG trigger injection, prior to ovulation, so that the sperm are in the uterus, waiting for the egg. The process is actually quite short, maybe only a few minutes from start to finish.
IUI's are not usually painful, you may experience some slight cramping (similar to the cramping felt during your period) both during and after the procedure. The only other discomfort most comment on is the discomfort of the speculum.
After the procedure is completed, you will be instructed to lay flat on the exam table for 20-30 minute then you are given the ok to leave. Most doctors suggest you go home and rest for the remainder of the day, and it is a good idea. One of the most common questions is "will I have to lay down the rest of the day? Won't "it" leak out?" The answer is NO. The semen will not "leak" out because your cervix is completely closed.
Some doctors will suggest you have intercourse following an IUI to insure you timed it correctly, and that there are some sperm there waiting for the egg. If your husbands sperm count is good, you can have intercourse every other day during your fertile period, and this, in theory, increases your chances of conception because you are insuring that one of those little guys catches that egg !!
Just like any other cycle, you will be able to test for pregnancy 2 weeks after your ovulation date. IUI's are not the same as IVF. IVF is a very aggressive infertility procedure often performed when the previously mentioned protocols have not worked, or when suggested by an OB/GYN or RE if pre-existing conditions exist.
An IUI is just another addition to the already detailed and busy medical protocol used to help women with PCOS conceive.

I've been experienced with IUI twice in my life.. But unfortunately, both unsuccessful..
That's make us frustated!! It feel like no more chance for us to have our own children.. Suddenly, one of my friends suggest us to do IVF..
Xtually, thats was a tough decision to make!! After doing some research regarding IVF treatment in M'sia, and last we found a centre that offer an affordable price & at the same time it doing collaboration with goverment hospital.. That's make us feel better & excited to do it..
In early 2008, i start my IVF fresh cycle and thank god, on Oct 2008 i've delivered my lovely twins safely!! That's was so miracle..
So, to all mommies wanna be, don't ever give up!! Keep trying & praying!! Wish u all da BEST & hopefully all your DREAM comes true!!
P/s: the centre that i mention above is 'Metro Maternity Hospital'
.. More details about the centre at: www.metro.com.my (all about infertility treatment) or www.metromaternity.com (all about services at the centre)

Hi,
I have tried 5 IUI at three gynae without success. They said I have unexplained infertility and that IUI can help. Then the last doctor I saw advised me to do IVF because the chances of successful pregnancy after two failed IUI is poor. I din try IVF earlier because of cost. I heard from friends that Metro Maternity Hospital in Klang has good result with IVF, but their cost for a cycle is still high for me, at 12k for one cycle.
Does anyone know of other good centers in Klang that do IVF at affordable price?

Hi LidaLay
The price for one IVF cycle is around 12k to 20 k. Of course its higher in KL. If you want cheaper center you can try KLGH or LPPKN. The cost is less. Klang GH also have IVF treatment at an affordable price around 7.5k. It was started in 2008 as a result of collaboration between Metro IVF and Klang GH. First part of treatment is at GH and second part at MetroIVF. There is a waiting time. There is also TAFF which is free.
I was like you few years back. I had 3 IUI and one IVF in PJ. These treatment cost us on my time and saving. I have to take leave at times. My hubby and I was looking for an affordable center in KL,PJ, Klang and Penang. Last year i tried IVF cycle at Metro IVF because of recommendation from my relatives in Klang. I m from Ampang. During the treatment time I had a lot of anxiety. Fear of failure again, whether the choice center was right or not. Or should I stop trying and use the money for other purposes. All these feeling disappear when dr Elizebeth shown us the pregnancy in the scan. The feeling was beautiful and more so when the baby was delivered early this year.
I hope these will help you to decide. But dont give up. A good IVF center has a cost.

MarySoo, Thank u very much for the info. You are very helpful. I am happy to hear u finnally have the baby u have waited. The period of anxiety and fear that u feel while waiting was worth it. U did the right thing.
I agree v u that good IVF center has a cost. I m not looking for a cheap center but for a affordable center. I heard from a friend who have IVF that her final cost was higher then initially stated. Cos she had repeated IVF and extra drug used. I am just careful. How much is your final cost? Is Metro Maternity Hospital and Metro IVF related. You were treated by dr Elizabert, how is she?

Hi LindaLay, Your planning on the cost for your IVF treatment is a good thing to do. Cost can change unexpectedly. Let me share my experience with u on this.
The cost for my IVF package was 12k. Then half way through my cycle, my eggs din develop and dr said it is because my age is 40 plus. Dr had to increase my drug dosage and duration for another one week. That's was the time I started to have a lot of anxiety. I thought I had made a wrong decision. And at the time of egg pick up, I had only two eggs available and after two days the embryologist told us that I have only one good embryo that developed. The fear of failure occurring to me again was like almost certain. It was stressful then. My hubby and me really appreciate the doctors and embryologists who are very supportive and encouraging. She told me; "I m lucky to have one good embryo at my age".
Then, suddenly all these fear disappear when dr showed us the pregnancy sac. It was good feeling and this good feeling becomes beautiful the following week when we were shown the baby's heart. It cost us 18k because i needed a lot more drug and longer stimulation time. I consider myself very lucky to be successful on the first IVF cycle. At my age I don't think I can have another pregnancy.
Metro IVF is in Metro Maternity Hospital Klang. They also have a branch in KL. I recommend u try at Metro IVF.
I hope my sharing will help you.
